Patient Participation Group Application Form

We have an active Patient Participation Group, also known as a PPG, which meets quarterly.

The PPG is made up of patients and GP practice staff who meet to discuss practice issues and the patient experience, helping us improve the service we provide.

The purpose of the PPG

The Patient Participation Group helps patients and practice staff work together to improve services.

To allow patients and practice staff to meet and discuss topics of mutual interest.
To provide a way for patients to become more involved and make suggestions about the healthcare services they receive.
To explore issues from patient surveys, contribute to action plans and help monitor improvements.
To provide feedback to the practice on patient survey results by proposing developments or changes.
To support health awareness and patient education.

Who can join?

The PPG is open to every patient on the GP practice list.

The group aims to include a broad range of representatives from different community groups, genders, ages, ethnicities and disabilities.

There are no other membership requirements, except that patients must be registered with the practice.
