The practice complies with all relevant data protection and access to medical records legislation.
How We Use and Share Your Information
Identifiable information about you will only be shared in the following circumstances:
- To support your medical care, for example with district nurses or hospital services
- To help you access other services, such as social services (with your consent)
- When we have a legal duty, such as in safeguarding or child protection cases
- Anonymous (unidentifiable) information may be used to help the NHS and Government plan services, for example for diabetes care
If you do not wish your anonymised information to be used in this way, please contact the practice.
Welsh GP Record
Your Welsh GP Record is a summary of key information from your full GP record. It is available to selected healthcare professionals, including Out of Hours services and some hospital teams.
This record may include:
- Your name, address and contact details
- Current medication and prescriptions from the last two years
- Allergies or adverse reactions
- Current health problems or diagnoses
- Results of recent tests (e.g. blood tests, X-rays)
It does not include private discussions you may have had with your GP.

Management of Records
Managing records and information is a key part of our Information Governance responsibilities.
This ensures we have the right information available at the right time to support safe and effective care.
We follow all relevant legislation, including:
- UK General Data Protection Regulation (UK GDPR)
- Data Protection Act 2018
- Freedom of Information Act 2000
Records may include:
- Paper and electronic documents
- Emails and correspondence
- Audio or video recordings
- X-rays and clinical images
- CCTV recordings
